Billing Info

It is our policy to file insurance claims quickly and accurately. It is important for you to understand that your insurance coverage is a contract between you, your employer and your insurance carrier. All questions regarding coverage should be addressed to them. There have been fast changes in benefits provided, frequency limitations, deductibles, etc. Therefore, it is important that you understand your policy so that we are able to file your claim.

We accept most major insurance plans as a regular service to our patients.  All we ask is that you present your insurance card at each visit so that we can verify the correct billing for your visit.  You are responsible for providing us with the correct billing information. 

Please be advised that you are responsible for any and all co-pays, deductibles and outstanding balances at the time of service.

PATIENTS WITHOUT INSURANCE COVERAGE:

Payment at the time of service is required.
